EPUT: Carers' Intensive Support Team
EPUT: Carers' Intensive Support Team provide a range of services to support vulnerable and ageing carers living in Southend, Castle Point, Rayleigh or Rochford (South East Essex). This includes: Identifying and supporting carers living with long term conditions; supporting carers in crisis and providing personalised health and wellbeing checks to help carers manage caring responsibilities. Referrals for this service are only accepted from professionals (no self-referrals).
If you or the person you care for require urgent support please contact: Your own GP, Dementia Intensive Support Team (DIST) 01268 739183; Community Coordination Centre 0333 015 3481; Physical Health concerns 111; Mental Health crisis 111 option 2, or Emergency 999.
We support vulnerable and ageing carers who are caring for:
- A person over 18 with suspected or confirmed dementia
- A person over 65 with physical health (frailty) and long term conditions
- A person over 65 with mental health concerns

Who can use this service?
How are services delivered?
This service is available to vulnerable and ageing carers (65+) living in Southend, Castle Point, Rayleigh or Rochford (South East Essex). If the carer is caring for someone with dementia, then the carer can be any age.
The team will contact the carer and carry out a home visit or if preferred meet at one of our Carers Hubs or Carers Groups (information is supplied on request).
